1. How do I refer a patient to your clinic?
Referrals can be sent via fax or secure email. Please include relevant medical history, imaging, and any specific therapy instructions or precautions.
2. What information should I include in the referral?
Please include:
- Patient demographics and insurance information
- Diagnosis and ICD-10 code
- Prescription or treatment recommendations
- Surgical protocol (if applicable)
- Imaging reports or clinical notes
3. How will I be informed of my patient's progress?
We provide timely documentation updates, including:
- Initial evaluation report
- Progress notes (typically every 4–6 visits or as requested)
- Discharge summary
We can also provide updates via phone or secure messaging upon request.
Treatment Approach & Clinical Coordination
4. What types of conditions do you treat?
We treat a broad range of conditions, including:
- Pre- and post-surgical rehab (e.g., TKA, THA, rotator cuff repair, ACL)
- Sports injuries
- Spinal conditions (disc herniation, stenosis, etc.)
- Overuse injuries (tendinopathy, bursitis)
- Arthritis, fractures, and joint dysfunction
- Women’s health issues – pregnancy, postpartum, pelvic floor
- TMJ
- Postural dysfunction
- Geriatric conditions - osteoporosis, balance, fall prevention, general debility
5. Do you follow post-operative protocols?
Yes. We strictly follow physician-supplied post-op protocols and communicate with you regarding patient adherence and progress. We can also help develop custom protocols as needed.
6. Are your therapists licensed and specialized?
All our therapists are licensed and trained in physical therapy with over 15 years of average experience. Many have advanced certifications, such as:
- Board-certified clinical specialists: Sports and Geriatrics
- CEEAA (Certified exercise expert for the aging)
- Osteoporosis specialist: Bone-Safe exercise practitioner
- Herman and Wallace pelvic rehabilitation institute practitioners
- Dry needling

7. Do patients see the same therapist each visit?
We strive to provide consistency in your care by scheduling you with the same therapist throughout the entire course of treatment. However, there may be occasional circumstances—such as scheduling conflicts, therapist availability, or the need for a specific area of expertise—when the patient may work with a different therapist. Rest assured, all of our therapists communicate closely to ensure seamless, high-quality care tailored to your patients' needs.
Access & Scheduling
8. How soon can patients be scheduled?
We can offer appointments within 24–48 hours, with same-day slots often available for urgent cases. Priority scheduling is available for post-op and acute injuries.
9. Do you accept walk-ins or direct access patients?
Yes. Our clinic follows state direct access laws. Patients may be evaluated without a referral when allowed by insurance. If further medical management is required, we will refer them back to a physician.
10. What insurance plans do you accept?
We accept most major insurances including Medicare and commercial PPO/HMO plans. We can provide a full payer list upon request.
Practice Integration & Collaboration
11. Can you co-manage complex or high-risk patients?
Absolutely. We are happy to collaborate with you on patients with multiple comorbidities, surgical complications, or unique rehab needs.
12. Do you offer outcomes data or patient satisfaction metrics?
Yes. We track functional outcomes (e.g., using FOTO or other standardized tools) and patient satisfaction scores. We’re happy to share this data to support your quality initiatives.
